Med spas and plastic surgery

AEO for med spas, aesthetic clinics and plastic surgeons

AEO for med spas is the work of getting your clinic or practice named, and described accurately, when patients ask ChatGPT, Google AI Mode, AI Overviews, Perplexity, Gemini or Copilot where to get Botox, which filler suits them, what a treatment costs or which surgeon to trust. We work with owners and marketing leads at independent med spas, multi-location aesthetic groups, dermatology clinics and plastic surgery practices in the US, UK, Canada, Australia and the UAE. You get a baseline of the prompts your patients type by treatment and city, a map of the sources cited today, rewritten treatment and pricing pages that stay inside advertising rules, and monthly reporting tied to consultation bookings.

Quick answer

How does a med spa or plastic surgery practice get recommended by ChatGPT and AI Overviews?

AEO for med spas starts with the sources answers already use: RealSelf, Google Business Profile, Yelp, booking platforms and practice pages. Publish plain-text treatment, candidacy, recovery and price-range answers, state provider credentials, keep every profile consistent, allow OAI-SearchBot and PerplexityBot, and track a fixed prompt set monthly. No one can promise a citation.

  • Brand names and RealSelf take most aesthetic citations today, so an independent clinic competes on specific treatment, city and candidacy prompts.
  • Treatment pages that state who is a good candidate, recovery time and price ranges in text give assistants something accurate to quote.
  • Every page an assistant reads is still advertising, so FDA, FTC, state board and UK ASA rules shape what we write.
  • We report citation share and consultation bookings from AI referrals, not screenshots.

01

Who is AEO for med spas built for, and who should skip it?

It suits four kinds of aesthetic business. Independent med spas that want to be the named clinic when someone asks where to get lip filler or laser hair removal in their city. Multi-location groups whose AI answers mix up addresses, prices or which treatments each site offers. Plastic surgery and facial plastic practices whose patients research for months before a consultation and ask assistants for surgeon names. And dermatology clinics with a cosmetic arm that want treatment explainers cited rather than a marketplace's summary of them.

It is a poor fit for a clinic with no reviews, no Google Business Profile and a website that is a booking widget with a logo. Fix those first; our page on local businesses covers that groundwork. Practices whose work is mainly medical dermatology, fertility or primary care face stricter health content rules and a different source mix, covered on our healthcare page. And if your calendar is full and you have no capacity for new patients, spend the budget elsewhere.

02

Which prompts do aesthetic patients ask, and who wins them today?

Aesthetic prompts split into five types: local clinic prompts (best med spa in a city), treatment cost prompts, treatment comparison prompts, candidacy and safety prompts, and surgeon selection prompts. The table summarizes the top web results we checked on 19 October 2026 with a US-based search tool. It shows who holds the ground that answer engines search through query fan-out; it is not a record of any single ChatGPT answer.

The pattern is consistent. Local and surgeon prompts are held by marketplaces, booking platforms, data aggregators and sponsored roundups, with very few clinic websites. Cost prompts mix RealSelf with price explainer sites and a handful of practice blogs. Comparison prompts such as Morpheus8 versus microneedling are the exception: practice pages hold most of the top results, because only a clinic can explain how it actually performs a treatment. That is where an independent med spa can win first.

What do citation studies say about med spa AI answers?

They say the category is concentrated and that RealSelf is the gatekeeper. A vendor study by 5W AI Communications, its Med Spa and Aesthetic Medicine AI Visibility Index 2026 (published 18 June 2026, 65 patient-intent prompts across ChatGPT, Claude, Perplexity, Gemini and Google AI Overviews), found the top 25 brands held 95% of AI citation share in US aesthetic medicine, while roughly 11,500 facilities below the index shared under 5%. Botox and CoolSculpting scored highest, and RealSelf ranked third, ahead of every med spa chain.

The same study found AbbVie/Allergan and Galderma together held 80% of drug and device citation share. Two lessons follow. First, an independent clinic will not out-cite Botox on what Botox is; it competes on the questions brand sites cannot answer, such as price in your city, who your injectors are and what a visit involves. Second, because RealSelf sits between patients and practices in so many answers, a complete RealSelf profile with real reviews and answered questions is part of the work, not an optional extra.

04

What must a med spa or practice have to compete in AI answers?

A practice needs its facts in plain text on its own site, the same facts on the platforms assistants read, and a specific reason to be named. Most aesthetic websites fail the first test: prices sit in Instagram stories, injector credentials sit in a team photo, and treatment pages repeat the manufacturer's copy that thousands of other clinics also use.

Off-site, the list is short but strict. Google Business Profile with the right primary category and every treatment listed. RealSelf for surgeons and injectors. Yelp, which a vendor study by BrightLocal found among ChatGPT's leading local sources. Your booking platform's public listing. Local press, best-of awards and roundups, which is a digital PR task. BrightLocal's consumer research also found people using AI to find local services rose from 6% to 45% in a year, so these profiles now feed answers as well as maps.

  • One page per treatment, opening with who it suits, who it does not, downtime and number of sessions
  • A price page with ranges per unit, area or session, the date it was last updated and what changes the price
  • Provider pages naming each injector or surgeon, their license type and board certification where it applies
  • Fair comparison pages for the treatment pairs patients ask about
  • Location pages per clinic with hours, parking and the treatments offered at that site
  • MedicalBusiness or LocalBusiness and Physician schema markup with sameAs links to RealSelf, Yelp and Google profiles

What advertising rules limit AEO for med spas?

Every page an assistant quotes is still an advertisement, and aesthetic medicine is under active enforcement. In October 2025, the American Med Spa Association reported that FDA had sent nearly 100 cease-and-desist letters and thousands of notices to drug companies, telehealth platforms and medical spas, with particular attention on compounded GLP-1 weight loss claims implying FDA approval or equivalence to Ozempic or Wegovy, and on missing risk information. Pages about injectables and weight loss need fair balance of risks, contraindications and no implied approval. The FTC Consumer Reviews and Testimonials Rule (16 CFR 465, in force since 21 October 2024) bans fake, bought and AI-written reviews, so review requests must be clean.

State rules matter too. Texas House Bill 3749, Jennifer's Law, signed in June 2025 and effective 1 September 2025, requires elective IV therapy outside physician offices to be given by a physician, PA, APRN or RN under physician supervision, so an IV drip page must describe who administers it accurately. In the United Kingdom, the ASA states that Botox and other botulinum toxin injections are prescription-only medicines that cannot be advertised to the public under CAP Code rule 12.12; clinics may promote a consultation for lines and wrinkles instead. That changes how a UK clinic's pages are written and which prompts it can target. This is not legal or medical advice; your medical director and counsel have the final word.

06

Which aesthetic pages should be rebuilt first?

Start with the pages that settle a patient's question and that only you can answer precisely: the price page, the top five treatment pages by revenue, provider bios and one comparison page for the pair patients ask about most. For surgical practices, add a procedure page per signature procedure covering candidacy, recovery week by week and what the consultation includes.

Each page opens with a two-sentence direct answer, states the conditions that change it, names the clinician who reviewed it and shows a last-updated date. Our AEO content writing team writes in that answer-first format, and our technical AEO work checks that booking widgets, price sliders and before-and-after galleries do not hide the explanatory text from crawlers. OpenAI's crawler documentation says sites that block OAI-SearchBot are not shown in ChatGPT search answers, and a default Cloudflare bot rule can block it without anyone noticing, so we confirm access in server logs.

07

How do we measure AEO for med spas against consultations?

We measure what fills the calendar. Each month we run a fixed prompt set by treatment and city in ChatGPT, Google AI Mode, Google AI Overviews, Perplexity, Gemini and Copilot, and record whether you are named, how your prices and providers are described, which competitors appear and which URL is cited. Our AI visibility tracking service runs the same method for groups that only want measurement, and citation share is the headline number.

In GA4 we separate referrals from chatgpt.com, perplexity.ai, copilot.microsoft.com and gemini.google.com and follow them to booking starts, consultation requests and calls. Bing Webmaster Tools AI Performance adds Copilot grounding queries, and Search Console shows the queries behind AI Overviews visits. Where answers state a wrong price or a treatment you no longer offer, we trace the source and fix it, the same process we use for wrong AI answers in any industry.

08

What mistakes do med spas make with AI search?

The most common is copying the manufacturer. A treatment page that repeats the brand's own description gives an assistant no reason to cite you over the brand. The second is hiding prices; when patients ask what something costs and you publish nothing, the answer quotes a marketplace average instead. The third is unsupported superlatives such as best injector in the city, which you cannot substantiate and which an assistant has no independent source to confirm.

Others are easy to spot in any AEO audit of a clinic site: provider names and credentials that differ between the site, RealSelf and Google; closed locations still listed on booking platforms; galleries with no text explaining the case; and review drives that offer discounts for five stars. Google's own AI optimization guide says optimizing for generative AI search is still SEO, so a clinic with weak local SEO should fix that before anything else. If you are comparing agency quotes, our AEO pricing guide sets out published market ranges.

FAQ

Frequently asked questions

What is AEO for med spas?

It is answer engine optimization applied to med spas, aesthetic clinics and plastic surgery practices: making sure AI assistants name your clinic and describe your treatments, prices and providers correctly when patients ask where to go. It builds on local SEO, with more weight on treatment answers, RealSelf and profile consistency.

Can a small independent med spa show up in ChatGPT?

Yes, mostly for city, treatment and price prompts rather than broad questions about what a product is. A 5W AI Communications study found independents share under 5% of aesthetic citations, so the gains come from specific pages and complete local profiles, not from competing with Botox or RealSelf head on.

Does RealSelf matter for AI visibility?

Yes. The 5W Med Spa AI Visibility Index 2026 ranked RealSelf third of all brands in aesthetic AI citations, and RealSelf pages led several of the surgeon and cost results we checked. A complete profile with real reviews and answered questions is part of the work.

Should med spas publish prices for AI search?

Where your market's rules allow it, yes. Cost prompts are among the most common, and without a dated price page the answer quotes a marketplace average. Publish ranges and what changes them rather than a single promotional price.

Is AEO for plastic surgeons different from AEO for med spas?

The method is the same, but surgeon prompts lean on credentials, RealSelf profiles and procedure galleries, and patients research for longer. Surgical pages need more detail on candidacy, recovery and risks, and board certification claims must be exact.

Can UK clinics optimize Botox pages for AI search?

Not as promotion. The ASA treats botulinum toxin as a prescription-only medicine that cannot be advertised to the public in the UK, so clinics write about a consultation for lines and wrinkles instead and keep product names out of promotional copy. This is not legal advice.

Should a med spa website block AI crawlers?

Block training crawlers if you wish, but keep search crawlers allowed. OpenAI says blocking GPTBot only opts out of training, while sites that block OAI-SearchBot are not shown in ChatGPT search answers.

How long does AEO take for a med spa?

Page fixes such as price and treatment pages can show in prompt results within weeks. RealSelf reviews, local press and roundup mentions usually take a quarter or more, so we judge progress on the 90-day rerun of the baseline.

Can you promise our clinic will be recommended by AI assistants?

No one can promise a citation, and no assistant publishes how it chooses sources. We improve what answers draw on and report citation share and consultation bookings from AI referrals monthly.

What does AEO for a med spa cost?

We quote a fixed price after a free AEO visibility check, based on how many locations, treatments and pages need work. Our AEO pricing guide lists published market ranges if you want a benchmark first.
